After you have scheduled your initial appointment you will be asked to fill out an intake form. You may take time to fill this out during your initial session, however you will be charged for the full hour regardless of how long it takes you to fill out the forms. Or, you may download the form using the link below, fill the form out at home, and bring the completed form with you at the time of your initial appointment.
